This policy describes information practices for Pennsylvania Tint Exemption and the administrative coordination performed by MyEyeRx. Independent clinical providers may provide a separate privacy notice for information they receive in connection with a consultation.
1. Information collected
Depending on how you use the site, information may include:
- Name, email address, phone number, date of birth, and address
- Intake answers, symptoms, medical history, and documents you choose to submit
- Consultation state, order amount, payment status, and transaction identifiers
- Device, browser, IP address, security, and basic site-usage data
- Messages and support requests sent to the coordination team
2. How information is used
- Save and manage an intake request
- Coordinate scheduling, referral, document collection, clinical review, and follow-up
- Process an eligible payment and reconcile its status
- Answer support questions and send service communications
- Prevent fraud, card testing, spam, malicious uploads, and abuse
- Operate, troubleshoot, secure, and improve the service
- Meet applicable legal and recordkeeping obligations
3. Service providers and disclosures
Information is disclosed as reasonably necessary to operate and coordinate the service. Current core providers include:
- GoHighLevel/LeadConnector: customer relationship management, workflow coordination, contact records, and document storage.
- Stripe: hosted payment checkout and payment records. Card details entered on Stripe Checkout are provided directly to Stripe rather than this site.
- Vercel: website hosting, delivery, and operational request logs.
- Independent licensed providers: information needed for an individual clinical review.
- Security, communications, file-scanning, and infrastructure providers used to protect and operate the service.
Information may also be disclosed when required by law, to protect rights or safety, in a business transaction subject to appropriate safeguards, or with your direction or consent.
4. Cookies and similar technology
The site and its providers may use cookies, local storage, and similar technologies needed for security, preferences, checkout, and basic performance measurement. Browser controls can limit some technologies, although doing so may affect site functions.
5. Data retention
Records are retained for as long as reasonably needed for the purposes described above, dispute prevention, security, recordkeeping, and applicable legal obligations. Retention periods can differ by record type and provider.

This site is not directed to children under 13. A parent or legal guardian should contact the coordination team before submitting information for a minor.
